Job summary

Cascade Health in Oregon is seeking a skilled Hospice Registered Nurse to join the Home Team, providing patient-centered care in patients' residences. Responsibilities include evaluating patients, planning and implementing interventions, and delivering education to patients and families.

The role requires current Oregon RN licensure, CPR certification, and at least three years med/surg or one year of critical care experience.

Qualifications

• Current Oregon licensure as a Registered Nurse.
• Current CPR certification.
• Three years of recent med/surg experience, or one year of critical care experience.
• Comfortable with computers and documenting using a laptop.
• Dependable vehicle for visits, valid driver's license and insurable driving record.

Responsibilities

• Conduct patient evaluation, develop the plan of care, interventions, and re-evaluation.
• Administer treatments, including IV therapy.
• Deliver patient and family education.
• Work independently with good time management and sound judgment.
• Use computers to document patient records.
• Communicate clearly via verbal, written, telephone, and electronic means.
• Participate in conferences, staff meetings, and in-services.
• Move and transfer patients and use healthcare equipment.
• Develop relationships with referral sources and share on-call responsibilities.
