Job summary

Compassus is seeking a Hospice Registered Nurse to model our values and support patients and families in home-based hospice care. The role involves routine and emergency assessments, case management, and collaboration with the interdisciplinary team to provide compassionate, high-quality care.

The RN will educate patients and families, document care activities, and coordinate with IDT and physicians to optimize comfort and symptom management in alignment with the care plan.

Qualifications

• Active RN license in state(s) of employment.
• Minimum 1 year nursing practice; hospice/home health experience preferred.

Responsibilities

• Perform routine and emergency patient assessments per policy.
• Provide clinical and psycho-social services per plan of care.
• Document observations, treatments, and communication with IDT.
• Educate patient and family on care, disease, dying process, wound care.
• Coordinate hospice care with IDT to ensure quality of care.
• Complete and submit IDT narratives and time logs by 10 a.m. next day.
• Flex schedule and provide after-hours on-call support.
