Job summary

Compassus is seeking a Hospice Registered Nurse to join our team, providing routine and emergency assessments, education, and evaluative services in a home-based setting. You will work as part of the interdisciplinary team to meet patient and family goals, adjusting to agency staffing needs and providing after-hours support as needed.

Qualifications

• Associate or Bachelor's degree in Nursing required.
• Active and unencumbered RN license in state(s) of employment.
• Current CPR certification required.
• Hospice, oncology, home health and/or long-term care experience preferred.

Responsibilities

• Completes routine and emergency assessments as indicated by policy.
• Provides clinical and psycho-social services per plan of care.
• Records observations, treatments, and other pertinent information.
• Communicates with IDT and physicians regarding symptom management and response to care.
• Educates patient and caregiver on care needs, disease process, and symptom control.
• Case manages routine and emergency patient care to prevent unnecessary hospitalizations.
• Updates plans of care and communicates changes to relevant staff.
