Job summary

Gentiva is seeking an experienced Hospice Clinical Liaison to ensure efficient intake and referral processes, coordinate admissions, and strengthen relationships with referral sources and families.

You will visit hospitals and facilities daily, educate patients on hospice care, and support the care team with smooth handoffs. This role requires RN licensure and CPR certification, with BSN preferred.

Qualifications

• Licensed Nurse with 3–5 years in hospice care.
• BSN with 5+ years hospice experience preferred.
• Minimum of 3 years hospice experience.
• Current RN license to practice in the state of employment.
• CPR certification.
• Valid driver’s license and automobile insurance.

Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians, case managers, SNFs, and other professionals.
• Assess patient eligibility for hospice by reviewing records and conducting evaluations.
• Educate patients and families on hospice care and processes.
• Complete documentation and coordinate intake/admission while meeting all requirements.
• Serve as liaison between referral sources and hospice care team; ensure clear communication.
• Make daily visits to hospitals and facilities to stay visible with discharge planners and physicians.
• Build and maintain relationships with referral partners and seek new outreach opportunities.
