Job summary

AccentCare is seeking a RN Clinical Director - Hospice in Fall River, MA. This full-time role supervises the patient and family care team, manages scheduling and IDG care plans, and collaborates with site leadership to ensure superior clinical and administrative outcomes.

Qualifications include an associate degree (CHPN preferred), 3+ years in relevant nursing, at least 1 year of supervisory experience, and a valid CPR certification. Must hold a valid driver’s license to travel to all locations.

Qualifications

• Associate degree preferred.
• Certified Hospice Nurse (CHPN) preferred.
• Three+ years' experience in hospice, home health, medical surgery, oncology, or geriatric/patient population-specific nursing.
• Minimum of one year of supervisory experience.
• Must be a licensed driver who can travel to all business locations.
• CPR certification preferred.

Responsibilities

• Supervise direct patient and family care team members, with program site leadership.
• Handle scheduling, payroll, and other administrative duties.
• Ensure initial assessments and required reassessments are conducted timely and documented.
• Oversee IDG development, implementation, and review of care plans.
• Collaborate with leadership to mentor staff in patient care activities and documentation.
• Participate in Back to Bedside efforts to reinforce Case Management skills.
• Collaborate with site leadership to support staff education.
• Facilitate IDG meetings and provide ongoing support including triage and calls.
• Participate in or lead quality improvement activities and data collection.
• Reinforce and support internal and external communications.
