Job summary

AccentCare Newark, Delaware is seeking a Hospice Registered Nurse Team Director to lead the patient care team and oversee clinical operations at multiple sites.

The role requires supervisory experience, CHPN preferred, an Associate degree, and a valid driver's license; travel to all locations is expected. You will coordinate IDG meetings, provide staff mentoring, and ensure timely assessments.

Qualifications

• Associate degree preferred.
• Certified Hospice Nurse (CHPN) preferred.
• Three+ years' experience in hospice, home health, or related nursing areas.
• Minimum of one year of supervisory experience.
• Possess and maintain valid CPR certification while employed in a clinical role (preferred).
• Must be a licensed driver and able to travel to all business locations.
• Meet state regulations where services are provided.

Responsibilities

• Supervise direct patient and family care team members, in collaboration with program site leadership.
• Responsible for scheduling, payroll, and other administrative duties.
• Ensure all initial assessments and required reassessments are conducted by qualified team members within required time frames.
• Oversee IDG development and implementation of the care plan for each patient and family.
• Collaborate with program site leadership to mentor staff in patient care activities and documentation.
• Support Back to Bedside efforts and reinforce Case Management skills.
• Collaborate with site leadership to support staff education efforts.
• Facilitate IDG meetings and provide ongoing IDG support and quality calls.
• Participate in or lead quality improvement activities, including data collection and aggregation.
• Reinforce and support all internal and external communications.
