Job summary

Allina Health is seeking a Hospice Resource RN to support patients, families, and interprofessional teams across the hospice territory. The role requires travel and entails an 8-hour day shift with 1.0 FTE and every 4th weekend rotation. Occasional on-call duties apply.

You will provide clinical expertise in symptom management, care coordination, and serve as a clinical resource for staff during coverage needs. Union engagement is involved.

Qualifications

• 2+ years RN experience required.
• Bachelor's degree preferred.
• RN licensure must be active in MN or WI depending on state of hire.

Responsibilities

• Assess patient and family needs and plan care with measurable outcomes.
• Maintain competencies for high quality patient care.
• Apply evidence-based nursing practices within regulations.
• Collaborate with family and patient to set realistic outcomes.
• Coordinate and delegate to ensure safe, effective care.
• Support charge nurse and preceptor roles as needed.
