Job summary

BJC HealthCare is seeking a Hospice RN Case Manager to deliver compassionate, patient-centered end-of-life care in home and community settings. You will manage a caseload of hospice patients, perform comprehensive assessments, and develop individualized care plans.

You will coordinate services across an interdisciplinary team, provide expert pain and symptom management, educate families, and document care in alignment with BJC’s standards of clinical excellence and mission-driven values.

Qualifications

• RN license required?
