Job summary

Affinity Hospice is seeking a Registered Nurse to join our Savannah, GA territory as a dedicated case manager providing hospice care in patients' homes and communities. The role emphasizes clinical leadership, family education, and care coordination to meet each patient’s goals.

The RN will work with interdisciplinary teams to implement patient plans, ensure safety, and document care while maintaining OSHA standards. A valid RN license, CPR, and driver with insured vehicle are required.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse
• Current CPR certification
• Minimum of two years of experience as a Registered Nurse in clinical care, home health or hospice setting preferred.
• Must be a licensed driver with insured vehicle and good working order.
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills.
• Able to lift, position and/or transfer patients.
• Able to lift supplies and equipment.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement/develop/document the patient plan of care.
• Provide care utilizing infection control measures that protect both the staff and the patient (OSHA).
• Assure continuity of quality patient care delivered with appropriate documentation.
• Monitor assigned cases to ensure compliance with requirements of third-party payer.
• Demonstrate commitment, professional growth, and competency.
• Promote agency philosophy and administrative policies.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Responsible for supervising hospice aides and licensed practical nurses.
• Perform other nursing duties as assigned by management to support department goals and initiatives.
