Job summary

Affinity Hospice seeks a Registered Nurse to serve as case manager, delivering compassionate hospice care in Savannah, GA. The RN will organize and direct services, educate families, and coordinate with the interdisciplinary team to meet patient needs in home and community settings.

The role covers the Savannah territory, involves on-call responsibilities, and requires an active RN license with CPR. A driving requirement and excellent communication are essential for success.

Qualifications

• Graduate of an accredited nursing program (Diploma/ADN/BSN).
• Current RN license and CPR certification.
• Minimum two years of RN experience in clinical care, home health or hospice.
• Must have a valid driver’s license and insured vehicle.
• Excellent observation, verbal and written communication, and problem solving.
• Able to lift and transfer patients and supplies.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Develop and document patient plan of care and use infection control measures.
• Ensure continuity of quality patient care with proper documentation.
• Monitor cases for third-party payer requirements and compliance.
• Provide on-call coverage and support for patients and families.
• Supervise hospice aides and licensed practical nurses.
