Job summary

Affinity Hospice seeks a Registered Nurse to serve as case manager, delivering compassionate hospice care in Savannah, GA. The RN will organize and direct services, educate families, and coordinate with the interdisciplinary team to meet patient needs in home and community settings.

The role covers the Savannah territory, involves on-call responsibilities, and requires an active RN license with CPR. A driving requirement and excellent communication are essential for success.

Qualifications

• Graduate of an accredited nursing program (Diploma/ADN/BSN).
• Current RN license and CPR certification.
• Minimum two years of RN experience in clinical care, home health or hospice.
• Must have a valid driver’s license and insured vehicle.
• Excellent observation, verbal and written communication, and problem solving.
• Able to lift and transfer patients and supplies.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Develop and document patient plan of care and use infection control measures.
• Ensure continuity of quality patient care with proper documentation.
• Monitor cases for third-party payer requirements and compliance.
• Provide on-call coverage and support for patients and families.
• Supervise hospice aides and licensed practical nurses.

Position Summary:

The registered nurse/case manager plans, organizes, directs, and provides hospice care and is experienced in nursing, with emphasis on community health education and experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.

Hours / Schedule:

Full Time; (40) hours / week. This position will cover our Savannah, GA territory.

Minimum Qualifications:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse Current CPR certification Minimum of two years of experience as a Registered Nurse in a clinical care, home health or hospice setting preferred.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
• Able to lift, position and/or transfer patients.
• Able to lift supplies and equipment.

Key Responsibilities:

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement/develop/document the patient plan of care Provide care utilizing infection control measures that protect both the staff and the patient (OSHA).
• Assure continuity of quality patient care delivered with appropriate documentation.
• Monitor assigned cases to ensure compliance with requirements of third-party payer.
• Demonstrate commitment, professional growth, and competency.
• Promote agency philosophy and administrative policies.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Responsible for supervising hospice aides and licensed practical nurses.
• Perform other nursing duties as assigned by management to support department goals and initiatives.
