Job summary

Affinity Hospice Holdings, LLC is seeking a Registered Nurse/Case Manager to plan, direct, and provide hospice care in home and community settings. The role emphasizes nursing expertise, care coordination, and education for patients and families.

Qualified candidates must hold an active RN license in Georgia, CPR, and have at least two years of clinical/hospice experience. A valid driver's license and reliable vehicle are required.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse (GA).
• Current CPR certification.
• Minimum of two years of RN experience in clinical care, home health or hospice.
• Must have a licensed driver with insured vehicle in good condition.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement/develop/document the patient plan of care.
• Provide care utilizing infection control measures that protect staff and patient (OSHA).
• Assure continuity of quality patient care with proper documentation.
• Monitor cases to ensure third-party payer requirements are met.
