Heart of Hospice in Fort Smith, AR, part of LHC Group and the Optum family, seeks an RN Case Manager to oversee the Plan of Care for hospice patients and coordinate with the IDG. You will develop treatment strategies and support physical, psychosocial, emotional, and spiritual well-being.

Responsibilities include an initial evaluation within 48 hours, plan development, documentation, and family guidance. Requires RN licensure and a valid driver’s license; CPR within 90 days preferred.

Qualifikationen

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• 1+ years of clinical experience (preferred).
• Current CPR certification or ability to complete within 90 days of hire.

Aufgaben

• Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
• Identifies the patient/family needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and patient/family response to care.
• Collaborates with the patient/family, attending physician, and IDG members for daily care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
