Heartlite Hospice, a part of LHC Group within the Optum family, is seeking a Registered Nurse Case Manager to oversee hospice patient care plans and collaborate with clinicians to promote comfort and dignity at home.

Requires an active RN license in the practice state, a valid driver's license with reliable transportation, and strong documentation and communication skills. You will evaluate patients, initiate and coordinate the plan of care, and educate families in home settings.

Qualifications

• Must hold current and unrestricted RN licensure in the state of practice.
• Valid driver's license, proof of vehicle insurance, and access to reliable transportation.

Responsabilités

• Make the initial nursing evaluation to determine eligibility for hospice within 48 hours of referral.
• Identify patient/family needs and reassess at least every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and patient/family responses to care.
• Collaborate with patient/family, attending physician and IDG members to provide daily care.
• Instruct and supervise patient/family in self-care techniques when appropriate.
• Maintain accurate and relevant clinical notes.
