Job summary

UnitedHealth Group, through Baptist Trinity Hospice, a part of LHC Group, seeks a Registered Nurse Case Manager to assume full nursing responsibility for the Plan of Care and collaborate with an IDG to deliver compassionate hospice care.

You will perform evaluations within 48 hours of referral, reassess every 15 days, and coordinate treatment with the physician and team. Sign-on bonuses: $10,000 for Hospice RNCM experience, or $5,000 if no Hospice background.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral to determine hospice eligibility.
• Identifies patient/family needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care with appropriate documentation.
• Collaborates with patient/family, attending physician and IDG daily.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes and adapts to home care settings.
