Job summary

Heart of Hospice, a part of LHC Group, is seeking a Registered Nurse Case Manager to oversee the Plan of Care for hospice patients. You will perform initial nursing evaluations, collaborate with the IDG, and update treatment plans to support physical, psychosocial, and spiritual well-being.

From Louisiana, the role requires RN licensure, a valid driver's license and access to a dependable vehicle. A $7,500 sign-on bonus is offered for full-time external candidates, with a comprehensive benefits

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Makes initial nursing evaluation within 48 hours of referral to determine eligibility for hospice services.
• Identifies physical, psychosocial, and environmental needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care; documents goals, interventions and responses.
• Collaborates with patient/family, attending physician and IDG daily for care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
