Job summary

Optum is seeking an experienced Registered Nurse Case Manager to deliver the Plan of Care for hospice patients. You will evaluate performance and collaborate with the IDG to implement treatment strategies supporting physical, psychosocial, emotional, and spiritual well-being.

The position requires a current RN license in the state, CPR certification, and at least one year of clinical nursing experience. A valid driver's license and reliable transportation are also needed.

Qualifications

• Active RN license in the applicable state.
• At least 1 year of clinical nursing experience.
• CPR certification required.
• Valid driver's license with reliable transportation.
• Ability to work with diverse patients and families.

Responsibilities

• Conduct initial nursing evaluation within 48 hours of referral.
• Identify patient/family needs and reassess every 15 days.
• Initiate and coordinate the plan of care.
• Document patient progress and care interventions.
• Collaborate with IDG to provide daily patient care.
• Instruct families in self-care techniques.
• Maintain accurate clinical notes.
