Job summary

Crescent Hospice is seeking a Full Time Hospice RN Case Manager to deliver hands-on nursing care and coordinate services across the Tri-County area (Berkeley, Charleston, Dorchester). You will guide patients and families through every step of the hospice journey with compassion and clinical excellence.

As part of our interdisciplinary team, you will educate families, support care planning, and participate in call duties.

Qualifications

• Hold current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice a plus.
• Proficient with computers and EMR systems.
• CPR certification as required by state.
• Valid driver's license and dependable transportation.

Responsibilities

• Deliver hands-on hospice nursing care and coordinate services.
• Guide patients and families through every step of the hospice journey.
• Collaborate with the interdisciplinary care team to plan and adjust care.
• Provide education and support to patients and families.
• Participate in call duties as part of the team.
• Ensure care plans reflect goals and comfort for patients.
