Job summary

Crescent Hospice is seeking a Full Time Hospice RN Case Manager to join our nursing team delivering compassionate end-of-life care throughout the Augusta area. The role includes hands-on clinical nursing, care coordination, and family education, with a $5,000 sign-on bonus.

Schedule is Monday–Friday, 8:00 a.m.–4:30 p.m., with a focus on clinical excellence, patient comfort, and teamwork within an interdisciplinary care model. An RN license and EMR proficiency are required.

Qualifications

• RN license active and unencumbered.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice a plus.
• Proficiency with computer systems including EMR.

Responsibilities

• Respond to clinical referral information with care and expertise.
• Partner with Clinical Director to assess eligibility for hospice services.
• Deliver hands-on hospice nursing care and coordinate services.
• Provide compassionate nursing care to ease pain and improve quality of life.
• Educate patients and families to create care plans aligned with goals.
• Share call duties as part of the team.
