Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager for hospice services within a home-care setting. You will evaluate and develop care plans, coordinate treatment strategies with the care team, and ensure high-quality, compassionate care for patients and families.

Responsibilities include initial assessments, ongoing care coordination, and documentation of patient progress, with emphasis on delivering holistic care and supporting families through challenging times.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identifies patient's physical, psychosocial, and environmental needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and patient/family response to hospice care.
• Collaborates with patient/family, attending physician and IDG to provide daily care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes regarding the patient's condition.
• Function in diverse home environments regardless of patient background.
