Job summary

Crescent Hospice is seeking a Full Time Hospice RN Case Manager to deliver hands-on care and coordinate services across the Tri-County area (Berkeley, Charleston, Dorchester). This role supports patients and families through the hospice journey with compassion, clinical excellence, and strong teamwork.

Sign-on bonus of $7,500 is offered. Schedule is Monday–Friday, 8 am–4:30 pm, with a comprehensive benefits package and opportunities for growth within a mission-driven organization.

Qualifications

• Current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice exp a plus.
• Proficient with EMR systems and computer skills.
• Current driver’s license and reliable transportation.

Responsibilities

• Empower Through Engagement: Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
• Collaborative Care Champion: Partner with Clinical Director to assess eligibility and suitability of clients for hospice services.
• Nurturing Direct Care: As an RN Case Manager, deliver nursing care and coordinate services.
• Nursing with Heart: Provide care rooted in proven nursing principles and techniques.
• Educator and Advocate: Educate patients and families to participate in personalized care plans.
• Call of Compassion: Share call duties as part of the team.

Crescent Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Schedule:

Monday - Friday, 8 am - 4:30 pm

Territory:

Tri-County - Berkeley, Charleston and Dorchester

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

$78,000 - $83,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
