Job summary

Crescent Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our nursing team providing exceptional end-of-life care across the community. Schedule: Monday - Friday, 8 am - 4:30 pm; Territory: Darlington, Lee and Chesterfield County; $7,500 Sign On Bonus.

You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey with the interdisciplinary team to ensure The Best Care Possible—compassion,

Qualifications

Hold current unencumbered license as a Registered Nurse.
Minimum one year experience as an RN in a medical-surgical/acute care setting; hospice a plus.
Must be computer proficient with EMR systems.
Current CPR certification if required by state.
Valid driver's license, vehicle insurance, and reliable transportation.

Responsibilities

Respond to clinical referral information with care and expertise, addressing inquiries compassionately.
Collaborate with Clinical Director to assess eligibility for hospice services.
As an RN Case Manager, coordinate care while prioritizing comfort and quality of life.
Deliver nursing care rooted in proven principles to ease pain and provide solace.
Educate and advocate for patients and families to create personalized care plans.
Share call duties as part of the care team.
