Job summary

St. Mary’s Home Care & Hospice in Athens, GA seeks a full-time Hospice RN Case Manager to oversee case management for hospice patients and coordinate with an interdisciplinary team. A $10K sign-on bonus is offered, with no on-call or weekend requirements.

Ideal candidates are RNs with Georgia licensure and at least 2 years of acute care, home care, or ER experience; EPIC experience is preferred. This role focuses on compassionate, patient-centered care within a supportive leadership structure.

Qualifications

• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in the State of Georgia.
• 2 years experience as a professional nurse in acute care, home care or ER experience required.
• Home care experience preferred but not required.
• EPIC experience preferred.

Responsibilities

• Case manage a team of hospice patients.
• Collaborate with interdisciplinary team and attend weekly Interdisciplinary meetings.
• Interact with the primary care physician and/or hospice medical director to manage patient symptoms.
• Position is full time primarily Monday-Friday with no on-call or weekend duties.
