Job summary
Crescent Hospice is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to join our compassionate nursing team serving Conway and surrounding communities. You will deliver hands-on hospice care, coordinate services, and guide patients and families through their hospice journey.
This role requires an active RN license, at least 1 year in acute care, CPR certification, and reliable transportation. We offer a $15,000 sign-on bonus, competitive pay, and a comprehensive benefits package.
Qualifications
- Hold current unencumbered license as a Registered Nurse.
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
- CPR certification current if required by state.
- Valid driver's license and reliable transportation.
Responsibilities
- Empower through engagement by responding to referrals with compassionate care.
- Collaborate with Clinical Director to assess eligibility for hospice services.
- Provide direct care as an RN Case Manager focusing on comfort and quality of life.
- Deliver nursing care rooted in evidence-based principles.
- Educate and advocate for patients and families to personalize care plans.
- Share call duties as part of the team.
