Job summary
Crescent Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team serving the Conway community. Expect schedule Mon-Fri 8:00–4:30 with on-call rotation; a $15,000 sign-on bonus and comprehensive benefits are included.
You will deliver hands-on hospice care, coordinate services, educate families, and guide patients through their hospice journey as part of our interdisciplinary team, committed to dignity, comfort, and compassionate connection.
Qualifications
- Active, unencumbered RN license.
- Minimum 1 year RN experience in med-surg/acute care; hospice exp a plus.
- Proficient computer skills and EMR experience.
- Valid driver's license and reliable transportation.
Responsibilities
- Respond to clinical referrals with care and expertise.
- Collaborate with Clinical Director to assess client eligibility.
- Nurture direct patient care and comfort as RN Case Manager.
- Deliver care using proven nursing principles.
- Educate families to participate in care plans.
