Crescent Hospice is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to join our compassionate nursing team serving Conway and surrounding communities. You will deliver hands-on hospice care, coordinate services, and guide patients and families through their hospice journey.

This role requires an active RN license, at least 1 year in acute care, CPR certification, and reliable transportation. We offer a $15,000 sign-on bonus, competitive pay, and a comprehensive benefits package.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
• CPR certification current if required by state.
• Valid driver's license and reliable transportation.

Responsabilités

• Empower through engagement by responding to referrals with compassionate care.
• Collaborate with Clinical Director to assess eligibility for hospice services.
• Provide direct care as an RN Case Manager focusing on comfort and quality of life.
• Deliver nursing care rooted in evidence-based principles.
• Educate and advocate for patients and families to personalize care plans.
• Share call duties as part of the team.

Are you a compassionate Registered Nurse (RN) who finds purpose in supporting patients and families through meaningful moments of care? Crescent Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Schedule: Monday - Friday, 8 am - 4:30 pm with On Call Rotation

Territory: Conway

• Empower Through Engagement: Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch that showcases our commitment to exceptional service.
• Collaborative Care Champion: Partner closely with our Clinical Director to assess the eligibility and suitability of clients for our specialized hospice services. Your expertise will guide us in providing the right care, at the right time.
• Nurturing Direct Care: As an RN Case Manager, you're not just coordinating care – you're a source of comfort and support for patients, ensuring their comfort and quality of life remain paramount.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

$78,000 - $83,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
