Job summary

Crescent Hospice is seeking a dedicated Registered Nurse Case Manager (RNCM) to join our compassionate nursing team. You will deliver hands-on hospice care, coordinate services, and guide patients and families through every step of their hospice journey.

In this role you will collaborate with the Clinical Director, provide education, and participate in on-call duties to ensure the highest level of comfort and quality of life for patients.

Qualifications

• Hold current unencumbered RN license.
• Minimum one year RN experience in medical-surgical/acute care; hospice experience a plus.
• Must be computer proficient, including EMR.
• Current CPR certification if required by state.
• Valid driver’s license, vehicle insurance, and reliable transportation.

Responsibilities

• Provide hands-on hospice nursing care and coordinate services.
• Collaborate with Clinical Director to assess client eligibility and care needs.
• Nurse with Heart: deliver care that prioritizes comfort and quality of life.
• Educate patients and families to participate in care planning.
• Guide families through the hospice journey and support appropriate decisions.
• Participate in on-call duties as part of a compassionate team.
