Allay Hospice is seeking a compassionate Hospice RN Case Manager to deliver patient-centered care with a strong nursing foundation. Responsibilities include planning and coordinating care, administering medications, and educating families in a supportive home setting.

The role offers a 40-hour work week, travel within the territory, and alignment with an interdisciplinary team. A driver’s license and insurability are required for local travel.

Qualifications

• RN licensure in the operating state is required.
• Minimum 1 year nursing experience; 3+ years preferred.
• Hospice experience preferred; knowledge of hospice philosophy.

Responsabilités

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient goals.
• Direct nursing care: medications, treatments, interventions.
