Job summary

Absolute Hospice is seeking an experienced Registered Nurse (RN) to join our hospice team as a Case Manager. The RN will provide direct patient care to hospice patients and support the patient’s family while coordinating an interdisciplinary team to implement and manage the plan of care.

Key duties include initial and ongoing patient assessment, education for patients and caregivers, and documentation per the Plan of Care. A $10,000 sign-on bonus is offered with full-time schedules.

Qualifications

• Active RN license in the state.
• Experience in hospice care and patient education.
• Ability to coordinate interdisciplinary care teams.

Responsibilities

• Provide direct patient care to hospice patients.
• Case manage patient clinical records.
• Educate patient, family and caregivers.
• Develop and implement the hospice plan of care with the physician.
• Assess patient needs and ensure high quality of care.
