Zusammenfassung

Allay Hospice, an affiliate of BrightSpring Health Services, invites a dedicated Hospice RN Case Manager to join a supportive team. You will provide patient-centered care at a critical time, coordinating with Social Work, Chaplaincy, Aides and clinicians to deliver meaningful, comprehensive hospice services.

Strong leadership and after-hours triage support are highlighted in this role. The position offers full-time hours (Mon-Fri, 8AM-5PM) with travel across the territory as required, primarily

Qualifikationen

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Aufgaben

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
