Job summary

Allay Hospice is seeking a compassionate RN Case Manager to deliver patient-centered hospice care in Wisconsin. You will join a strong interdisciplinary team and coordinate plans with Social Work, Chaplaincy, Aides, and clinicians, while traveling through the territory as required.

This full-time role offers a $5,000 sign-on bonus, dedicated after-hours triage support, and a comprehensive benefits package including health insurance, retirement, and mileage reimbursement.

Qualifications

• Graduate of an accredited nursing school with current RN license in the operating state.
• Minimum 1 year of nursing experience; 3+ years preferred; hospice experience preferred.
• Must have reliable vehicle and valid driver’s license.
• Knowledge of hospice philosophy of care and documentation excellence.

Responsibilities

• Assess and monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients' families and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
