Job summary

Allay Hospice is seeking a passionate RN Case Manager to join our team in a setting with lower patient-to-nurse ratios and strong interdisciplinary support. A $5,000 sign-on bonus underscores our commitment to attracting dedicated hospice professionals.

Responsibilities include patient-centered care, care plan development, and collaboration with social work, chaplaincy, aides, and providers. Travel within the territory is required; office hours are Monday–Friday, 8 AM–5 PM.

Qualifications

• Graduate of an accredited nursing school with active nursing licensure in the operating state.
• One year nursing experience required, 3+ years preferred.
• Hospice or similar setting experience preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with a vehicle and valid insurance.

Responsibilities

• Assess and monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligning with patient wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
