Job summary

Allay Hospice, an affiliate of BrightSpring Health Services, is seeking a Hospice RN Case Manager to deliver compassionate 1:1 care across a defined territory in Wisconsin. You’ll work with an interdisciplinary team to create personalized care plans, manage symptoms, and educate families, all while maintaining meticulous documentation.

The role offers a $5,000 sign-on bonus, full-time schedule, and travel within assigned counties.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in the state of operation.
• One year of nursing experience required; 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an insured automobile in good working order.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patients’ wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients’ families and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
