Job summary

Amedisys Hospice is hiring an RN Case Manager to coordinate compassionate, individualized hospice care. You will assess patients, partner with the interdisciplinary care team, and develop plans to support each patient’s needs.

This full-time role requires an active RN license in the assigned state, CPR certification, and at least 1 year of nursing experience. A valid driver’s license and reliable transportation are also required.

Qualifications

Current RN license valid for the state(s) assigned.
Active CPR certification.
Minimum 1 year of nursing experience.
Valid driver’s license and reliable transportation.

Responsibilities

Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
Develops and maintains individualized patient care progress records with accuracy and timeliness.
Records pain and symptom management changes/outcomes as appropriate.
Teaches patients, families, and caregivers appropriate patient care techniques.
Regularly communicates with the clinical manager and care team.
Performs on-call responsibilities and on-call services to patients/families as assigned.
Other duties as assigned.
