Job summary

Heart’n Soul Hospice is seeking an experienced RN Case Manager to join our team. The role focuses on comprehensive assessments, developing individualized plans of care, and coordinating care across physicians and healthcare professionals to support patients with life-limiting illnesses.

You will participate in IDG meetings, ensure timely documentation, and provide on-call coverage as needed while maintaining compliance with regulatory standards and company policies.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients with life-limiting illness.

• Provide on-call coverage and timely documentation of care.
