Job summary

Gentiva Hospice is seeking a dedicated Hospice RN / RN Case Manager to lead patient care in home settings. You will perform assessments, implement plans of care, and collaborate with physicians, LPNs, CNAs, social workers, and chaplains to tailor compassionate end-of-life care.

You will educate caregivers, document care activities, and participate in on-call services, ensuring timely interventions and adherence to regulatory standards.

Qualifications

• Current RN license in state of employment.
• Hospice or palliative care experience preferred.
• Experience delivering in-home nursing care and education.

Responsibilities

• Lead patient and family through the hospice journey.
• Deliver and document skilled nursing care per Plan of Care.
• Perform initial and ongoing patient assessments.
• Coordinate with IDG to refine care plans.
• Provide education and bereavement support to families.
• Maintain timely clinical documentation and compliance.
