Job summary

Horizons is seeking an RN to administer nursing care for patients in their homes, coordinating with the interdisciplinary team, patient, family and referring agency. The role emphasizes comprehensive assessments, care planning, and education on end-of-life care.

The ideal candidate will have a nursing degree (Diploma/Associate/Bachelor), an active RN license, and strong English communication; CPR certification is required. A sign-on bonus is available after 6 months of employment.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current RN license and valid driver's license.
• Ability to read, write and comprehend English.

Responsibilities

• Initial and ongoing comprehensive assessments of patient needs and environment.
• Develop and implement plans of care with the interdisciplinary team.
• Educate patients and families about disease process, end-of-life care and self-care.
• Coordinate care, communicate changes to physicians and staff, ensure documentation.
• Provide on-call coverage as assigned.
