Job summary

Heartland Hospice is seeking a dedicated RN Case Manager to lead in-home hospice care across patient families. You will assess, plan, and deliver skilled nursing services to patients in their homes, coordinated with an interdisciplinary team to ensure compassionate, comprehensive care.

The role emphasizes education for patients and families, symptom management, and documentation, with a sign-on bonus as part of the welcome package. Prior hospice or palliative care experience is preferred.

Qualifications

• RN license to practice nursing in the state of employment is required.
• CPR certification is required.
• Experience in hospice, palliative care, or home health is preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic visits.
• Deliver and document skilled nursing care aligned with the Plan of Care.
• Develop and update individualized care plans and visit frequencies.
• Coordinate care with IDG, physicians, and other team members.
• Provide education and bereavement support to patients and families.
