Job summary

Optum is seeking a Registered Nurse Case Manager in Crystal Lake, Illinois, responsible for delivering comprehensive nursing care for hospice patients. You will develop and implement treatment plans through collaboration with families and professionals, ensuring quality of care.

The position requires a valid RN license, CPR certification, and at least 1 year of clinical experience. A $2,500 sign-on bonus is offered. Attractive salary range is $60,200 to $107,400 annually, depending on experience.

Qualifications

• Must have 1+ years of clinical experience.
• Ability to function in any home situation.
• Current driver's license and valid vehicle insurance required.

Responsibilities

• Evaluate eligibility for hospice services within 48 hours of referral.
• Identify and reassess patient's/family's needs every 15 days.
• Collaborate with patient's family and IDG daily.
