Job summary

Sanctuary Hospice is seeking a Full Time Hospice RN Case Manager to join our nursing team delivering exceptional end-of-life care across Hoffman Estates, Barrington, Cary, Crystal Lake, Streamwood and Algonquin. You will coordinate services, guide patients and families, and collaborate with an interdisciplinary team in a compassionate, clinically excellent environment.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one year experience as an RN in medical-surgical/acute care; hospice experience a plus.
• Must be computer proficient with EMR; CPR certification may be required.
• Valid driver's license and reliable transportation.
