Job summary

Angels Care Hospice is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of a patient''s home care and collaborate with other medical disciplines. The role includes performing admission visits to determine eligibility and developing the plan of care to be followed.

Ideal candidates hold an active RN license, have 1 year of RN experience (2 preferred), CPR/Hepatitis clearance, and a valid driver''s license with reliable transportation.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver's License.
• One-year's experience as a Registered Nurse, two preferred.
• Proof of CPR and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient''s home care and interact with other medical disciplines as needed.
• Perform the admission visit to determine eligibility for home care services.
• Develop and follow the plan of care for each patient.
