Job summary

Angels Care Hospice in Texas is seeking a Registered Nurse Case Manager to coordinate all aspects of patient home care. You will perform admission visits, assess eligibility for home care services, and develop the plan of care in collaboration with the patient and family.

The role requires an accredited nursing diploma/degree, current RN license, valid driver's license, and at least one year of RN experience (two preferred).

Qualifications

• Graduation from an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver's License.
• One year of RN experience; two years preferred.

Responsibilities

• Coordinate all aspects of the patient's home care.
• Perform admission visits and determine eligibility for home care services.
• Develop and implement the plan of care in collaboration with patient and family.
