Job summary

Angels Care Hospice is seeking a dedicated RN Case Manager to coordinate all aspects of the patient’s home care, collaborating with other medical disciplines as needed to deliver professional, compassionate care in the patient’s home. You will perform admission visits, assess eligibility for home care services, and develop the plan of care.

The role requires RN license, nursing degree, CPR, valid driver’s license, and a year of RN experience; Angels Care supports ongoing training and career

Qualifications

• Must hold an active RN license and nursing degree from an accredited program.
• Current driver’s license and reliable transportation required.
• CPR certification and Hepatitis consent/declination required.
• Minimum 1 year RN experience; 2 years preferred.

Responsibilities

• Coordinate all aspects of a patient’s home care as an RN Case Manager.
• Interact with other medical disciplines as needed to provide care.
• Perform admission visit in the home and assess eligibility for home care.
• Develop and follow the plan of care for the patient.
