Job summary

Angels Care Hospice seeks a Registered Nurse (RN) Case Manager to coordinate all aspects of the patient’s home care, collaborating with other medical professionals to deliver high-quality nursing care in the patient’s home.

You will perform the admission visit, determine eligibility for home care services, and develop the plan of care to be followed. A current RN license, valid state driver’s license, and reliable transportation are required.

Qualifications

• Graduate of an accredited nursing program (Diploma/ASN/BSN).
• Current state RN license.
• Valid state driver’s license.
• One year RN experience (two preferred).
• CPR certification and Hepatitis consent/declination.
• Reliable transportation with auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care with other medical disciplines.
• Perform admission visit to determine eligibility for home care services.
• Develop and follow the plan of care for each patient.
• Collaborate with the care team to ensure high-quality nursing care at home.
