Job summary

Angels Care Hospice in Texas is seeking a dedicated Registered Nurse Case Manager to coordinate all aspects of a patient's home care. You will collaborate with medical teams as needed and conduct admission visits in the patient’s home to determine eligibility and plan of care.

Qualifications

• Graduate of an accredited diploma, associate, or baccalaureate nursing program.
• Current state RN license.
• Current state driver’s license.
• One-year’s experience as a Registered Nurse, two preferred.
• Proof of CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• Coordinate all aspects of home health care for patients.
• Interact with other medical disciplines as needed.
• Perform admission visit in patient’s home.
• Develop and implement plan of care.
