Job summary

Angels Care Hospice is seeking an RN Case Manager to coordinate all aspects of the patient’s home care and collaborate with the medical team. You will conduct admission visits, assess eligibility for home care services, and develop the plan of care to be followed.

Qualified candidates are graduates of an accredited nursing program, hold an active RN license, and have CPR certification. One year of RN experience is required, with two years preferred, plus a valid driver's license and reliable

Qualifications

• Graduate of an accredited nursing program with active RN license.
• Current RN license in the state and valid driver's license.
• Minimum one year of RN experience; two years preferred.
• CPR certification and Hepatitis consent/declination on file.
• Reliable transportation with insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care and collaborate with medical disciplines as needed.
• Perform admission visit to the home and assess eligibility for home care services.
• Develop and follow the plan of care in coordination with the care team.
