Job summary

Angels Care Hospice is seeking a Registered Nurse Case Manager to coordinate all aspects of the patient’s home care. You will interact with other medical disciplines, perform the admission visit, determine eligibility for home care services, and develop the plan of care to be followed.

Graduate of an accredited nursing program, current RN license, and valid driver’s license required. CPR certification and reliable transportation are also essential for success in this role.

Qualifications

Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
Current state license as a Registered Nurse.
Current state Driver's License.
One-year's experience as a Registered Nurse, two preferred.
CPR certification and Hepatitis consent/declination.
Reliable transportation with valid auto liability insurance.

Responsibilities

Coordinate all aspects of the patient's home care.
Interact with other medical disciplines as needed and provide professional care.
Perform admission visit to the patient in the home and determine eligibility for home care services.
Develop and follow the plan of care for each patient.
