Angels Care Hospice is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of in-home patient care. The RN will conduct admission visits, assess eligibility for home health services, and develop individualized plans of care in collaboration with other medical professionals.

The role requires current RN licensure, CPR certification and a valid driver's license, with at least one year of RN experience (two preferred).

Formación

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• One-year's experience as a Registered Nurse, two preferred.
• CPR certification and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsabilidades

• Coordinate all aspects of the patient's home care and interact with other medical disciplines as needed.
• Perform the admission visit to the patient in the home, determine eligibility for home care services, and develop the plan of care to be followed.
