Trustbridge Hospice, part of Empath Health, is seeking a compassionate Hospice Registered Nurse (RN) Case Manager to join our Palm Beach County team. Monday–Friday 8:30am–5:00pm with weekend requirement.

Trustbridge Hospice has served Palm Beach and Broward counties with Full Life Care since 1978, focusing on comfort, dignity, and family-centered support. We are a mission-driven, not-for-profit network under Empath Health.

Qualifications

• Registered Nurse licensed in Florida.
• Current Basic Life Support certification.
• One year in hospice, public health, home health, oncology, medical/surgical, or geriatric nursing preferred.
• Knowledge of hospice nursing: case management, crisis intervention, communication, teaching, thanatology.

Responsibilities

• Serve as a key advocate for patients and families, ensuring needs are recognized and addressed.
• Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team.
• Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group).
• Use clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life.
• Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust.

Trustbridge Hospice, member of Empath Health, is seeking a caring and motivated Hospice Registered Nurse (RN) Case Manager to join our dedicated hospice team in the Palm Beach County area. In this vital role, you will provide exceptional, patient-centered care while guiding families through one of life’s most meaningful journeys on the following schedule: Monday - Friday 8:30am - 5:00pm with a weekend requirement.

What You'll Do

• Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed


• Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team


• Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group)

What You’ll Need

• Registered Nurse currently licensed in the State of Florida.


• Current Basic Life Support Certification.


• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.


• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs


• Basic computer skills and EMR software experience a plus.


• Adequate and reliable means of transportation with adequate insurance and valid Florida driver's license.
