Job summary

Trustbridge Hospice, a member of Empath Health, is seeking an RN Case Manager in Boca Raton, FL. You will coordinate comprehensive hospice care, advocate for patients and families, and work with an interdisciplinary team to deliver comfort and education throughout the care journey.

The role requires Florida RN licensure, strong communication, and experience in hospice or related fields. This is a full-time position with weekday hours and alternate weekends.

Qualifications

• Licensed registered nurse in Florida in good standing.
• 1+ year in hospice, home health, oncology, medical/surgical, or geriatric nursing preferred.
• Strong communication and teaching skills to support patients and families.

Responsibilities

• Coordinate care under physician direction and with the interdisciplinary group.
• Advocate for patients and families and address their needs.
• Monitor patient conditions, provide interventions, and deliver education to improve comfort and quality of life.
• Address concerns promptly to maintain high satisfaction and trust.
